নমস্কার বন্ধুরা আজকের এই পোস্টের মাধ্যমে আপনাদের বলব যে বর্তমানে Swasthya Sathi Card অনলাইন আবেদন শুরু হয়ে গেছে এখন কিন্তু আপনি অনলাইনে মাধ্যমে স্বাস্থ্য সাথী কার্ড এর জন্য আবেদন করতে পারবেন | এবং আপনাকে অনলাইন আবেদন করার জন্য কোথাও যাওয়ার দরকার নেই আপনি বাড়িতে বসেই আপনার মোবাইল বা ল্যাপটপের মাধ্যমে খুব সুন্দর ভাবে আবেদন করতে পারবেন |

1. অনলাইন আবেদন করতে কি কি ডকুমেন্টস লাগবে?

1) AADHAR Card Number (12 Digits)
2) KHADYASATHI Ration Card Number
3) Registered Mobile Number

 

2. কিভাবে অনলাইন আবেদন করবেন?

Step – 1  https://swasthyasathi.gov.in/

Step – 2    Apply Online Online Application For Swasthya Sathi Card

Step -3     1) Provide your Mobile Number in the appropriate place and click the get OTP button
2) Put the OTP received in the proper place and submit

Step – 4  1) Select the appropriate District from the list
2) If you belong to Panchayat Area, choose your Block, Panchayat & Village name from the list
3) If you belong to Municipality Area, Choose your Municipality Name then choose your Ward Number
4) Choose the proper category you belong to from the list, SC / ST / OBC / Others
5) Fill up your Department Name If Employed
6) Fill up Office Name & Address If Applicant / Member is Employed
7) Fill up your Residential Address with the +-PIN Code
8) Fill up the Name of the Applicant
9) Fill up the Head of the Family (HoF) Name
10) If Any Member of the family receives Govt. Sponsored Health Insurance / Assurance, choose the option

from the list YES / NO
11) If Any Member of the family receives Medical Allowance from Govt. the, choose the option from the list YES                    / NO
12) If Any Member of the family receives Medical Allowance from Private Institution, choose the option

from the list YES /NO

 

3. কিভাবে স্ট্যাটাস চেক করবেন?

স্ট্যাটাস চেক করে জন্য প্রথমেই এই অফিসের ওয়েবসাইট ভিজিট করবেন –  Click Here 

তারপর রেফারেন্স আইডি নম্বর টা বসিয়ে দেবেন । যেইটা এপ্লিকেশন করার সময় আপনাকে দিয়েছিল সেই অ্যাপ্লিকেশন নাম্বারটা বসিয়ে আপনি আমার স্ট্যাটাসটা চেক করতে পারবেন ।
